Worst clinic I've been to for a DOT physical, Phyllis made every excuse in the book and denied me my DOT medical card. Even after denying me my DOT medical card and not even conducting a physical exam, they would not refund my $140. Which is extremely high for a DOT physical. Phyllis will screw you over, take your money and make excuses to why she "cant" won't issue you a DOT medical card.
