Paid $150 for the appointment. Filled out everything they asked for, and even called prior to my appointment to ensure that they had all the necessary paperwork and information. They told me everything was good. I pull up and sit down with the lady, who then proceeds to skim over the medical records, and questions my diagnoses for a few minutes.
