I received chiropractic services from Dr. Chamberjian in 2018. At that time I was informed by his office that I would be responsible for a co-pay of $30.00 a visit as they were an in-network provider. I paid that co-pay each visit. I later received a bill from him. After many months of investigation I was informed by my insurance company that Dr. Chamberjian submitted billing codes they did not cover, and I would have to address this bill with him.
