Did this provider seem up-to-date with the current advancements in their field? Yes Was this provider argumentative or easily angered? Not at all, they were one of the kindest, most attentive individuals I've ever met

Yes, they've stayed open late when I've needed help Did you leave the office feeling satisfied with your visit? Yes Do you feel that you could have received better service somewhere else? No, I don't think so Was this provider argumentative or easily angered? No, they were very calm and understanding

Did this provider have an entertaining selection of waiting room magazines? Somewhat, but they need a bigger selection Were the staff members able to answer your questions? Yes, they were able to answer my questions Was it easy to make an appointment with this provider? I waited longer than I wanted to, but it wasn't terrible Was there annoying music playing while you waited or during your visit? No, there was no annoying music

Do you feel that you could have received better service somewhere else? No, I don't think so Did this provider wear an obnoxious cologne? No, they didn't wear any cologne Are you confident that this provider will continue working with you until a solution is reached? Yes, I know I can count on them to find a solution

Is this provider's business conveniently located? Not at all, its out in the boondocks Did you have to wait long after you arrived for your appointment? I had to wait for a little while. Did this provider promise services he/she couldn't provide? No, they were able to provide everything they said they would Does this provider always put the customer first? Yes, they always put customers first

No, they never postpone my appointment Does this provider offer any coupons, discounts or promotions? Not that I have seen Does this provider ever overbook appointments here? No, They definitely believe in quality over quantity and allow more than enough time in between appointments
