Was there annoying music playing while you waited or during your visit? No, there was no annoying music Did you leave the office feeling satisfied with your visit? Completely! Were the restrooms clean at this provider's office? Yes, they were immaculate! Did this provider wear an obnoxious cologne? No, they didn't wear any cologne Does this provider always take that extra step to make you feel special? Yes, I always enjoy my appointments

Did this provider answer all of your questions? Every question I had was answered thoroughly Was this provider sympathetic to your situation? Yes! They went above and beyond to make me feel special and respected Did this provider use excessive foul language? Not at all! They are always respectful and courteous! Did this provider's staff seem to respect him/her? Definitely! They staff spoke very highly of him/her Is this provider's business conveniently located? Absolutely! It is right off the freeway and close to everthing

Yes everything was neat and sterile Did you have to wait long after you arrived for your appointment? No, I didn't have to wait very long Did this provider thoroughly explain the risks and benefits of your treatment? Yes, I felt informed when I left

Did this provider seem irritated to be working with you? Not at all, they were glad to help me with anything I needed Were the staff members able to answer your questions? Yes, they were able to answer my questions Did this provider listen to your input and concerns? Absolutely! This provider always listens carefully to what I have to say! Is this business handicap-accessible? Yes, it was handicap-accessible Do you feel that you could have received better service somewhere else? No, I don't think so
