I was referred to Dr. Valenzuela after having some issues post partum. I saw him once and I noticed he had a few errors in my visit notes they didn't add up to my medical records so I sent a respectful message asking it be changed. The issues included him spelling my name wrong not once but THREE times, stating I had a daughter which I do not, and stating I had issues that are not mine but in my family history.
