This Doctor blatantly mistreated me with disrespect to my disability and my ability. She came into the office 30 minutes late and without greeting me- instead informed me that we may not be able to complete my service due to timing and other clients. Then without looking at my medical form from MVA she refused to complete it without a letter from another specialist that have nothing to do with the actual Physical- Medical form. Did not get see because I was 10 minutes late and they have a 15 minute late policy. The reason I wasn’t seen because my pre-registration packet wasn’t complete prior to the appointment. This is the first doctor that refused to see me because I didn’t complete my pre-registration packet, it doesn’t say you will not be seen if not completed prior to appointment.
