On December 2, 2025 Supervisor Health Plan faxed a form to Dr. Lenz, which need to be returned ASAP. I started to call receptionist (office), they started giving me the run around told me they needed to see my mother but that was said to me till Thursday in order to fax form back to Supervisor. I took her that week on Friday. They Practiconer told me the form was going to be ready the following Monday to be faxed. I was getting more frustrated but waited patiently for Monday.
