Great doctor/patient skills. Knowledgeable. Also, respectful of my knowledge of my body. Not pushy.
These people are completely Incompetent, lost forms three times. Then when the Dr finally filled it out they did it wrong and there we direction in bold! The bold direction said use black ink, they used red. It also said don't attach any documents fill out the form. Left the form blank and didn't fill out anything. They had a month and three tries to get it right. I trust McDonalds more.
