THERAPIST HAS UNETHICAL AND DISHONEST PRACTICES. She charted a diagnosis of Bipolar disorder for a parent on a child's chart and never met with the parent once. The child was the patient, and the parent sat in the car. She won't remove the diagnosis, she lied multiple times on the chart and did not include communications about the child in the records, despite making reference to them being attached to the record, meaning she didn't fully disclose the record and altered it, on top of blatantly lying.
