Dr was 10 minutes late- door was locked and I had to wait outside for him He handed me a 20 page intake form & when I said I didn't have an answer for many of the questions, he said if the form is too stressful for me, then we should cancel. I explained that I have been in pain waiting for this appointment and he said,"Yeah- this isn't going to work out" Totally unprofessional. Totally insensitive.
