My husband was at facility twice. Doctor said he was ready to be dismissed. Dismissal time was set, went through procedure. Didn’t check until gone, got someone else’s clothes. Was called on way home, they forgot to get all check out info, so had to give over phone. Said I would be getting an opportunity to fill out comment form, never got!!
