You have to give your credit card number on the intake form, and if your insurance (mistakenly) does not pay she will charge your card. She did this to me and it took me months and many calls to the insurance company to have a refund sent to her. Now I have left many messages and emails to have her refund the amount $305 that she charged to my credit card. She did finally email me to let me that I did not understand the charges. I have sent her all the paperwork and now she will not call or email me back...
