The Dr recommended me for an in-clinic sleep study, which requires pre-authorization. They did not remember to actually submit that pre-authroization request until weeks later when I called. When they DID submit it, they didn't include the doctor's notes -- and it was subsequently denied. They did not tell me it was denied until I called days after the fact. I asked for an appeal.
