We have no issue with the health care services that Dr. Khalifa provided. The issue is with the office manager and the billing staff. All of our bills were paid through the insurance company and out of pocket fees were all paid as well. Except for one bill, service date: 9/29/2016, the staff submitted the wrong tax ID to the insurance company, so the insurance company declined the claim.
