I am writing this review to help other patients navigate Community Care and to share my experience coordinating care after a stroke, heart failure diagnosis, and worsening kidney disease. Unfortunately, I have experienced several communication and coordination issues. I was scheduled for a medication assistance appointment that was supposed to take place by phone, but I never received a call.
