Does the provider's staff try to be helpful? No Did this provider seem well-trained and experienced? Not at all Was it easy to find parking at this psychiatrist's office? Extremely easy, and close to the entrance Did this provider seem up-to-date with the current advancements in their field? No Was the provider's staff helpful and knowledgeable? No

Was this provider's office easy to locate? Fairly Easy Did this psychiatrist have reasonable prices? No, it was expensive. Did this psychiatrist have reasonable prices? Not at all, they were ridiculous! Was this provider's office too loud? Not at all, it was very peaceful and quiet Would you refer this provider to a family or friend? They always make an extra effort to be polite and courteous. I could tell that they took pride in their office's appearance. It was very well organized and well taken care of. The staff was very friendly and warm. They went out of their way to make me feel respected and cared for. Their office was well organized and clean. I could tell they took a lot of time making sure it was presentable. They always made sure that I was helped in any way possible.

Did this provider answer all of your questions? No, I left confused and frustrated Was this provider's staff friendly? Not Really Did you leave the office feeling satisfied with your visit? No, I left with more problems than I came with!

Yes, they were informing me of new advancements that were extremely recent Did this psychiatrist have reasonable prices? Yes Does this psychiatrist always take that extra step to make you feel special? Yes, I always enjoy my appointments Was this provider's office clean? Yes everything was neat and sterile

Did you have to wait long after you arrived for your appointment? Yes, I had to wait quite a while Did this provider ever postpone your appointment? No, they never postpone my appointment Did this psychiatrist give you undivided attention? Not really, they seemed somewhat distracted
