Did this provider show attention to detail? Definitely not. They were unaware of my situation, even though they had my information Was this provider's office too loud? No, it wasn't too loud Was the water they used during your teeth cleaning a comfortable temperature? Not really, it was a little uncomfortable Were you able to relax during your appointment? No, I was on edge Did this provider leave you unattended for an extended period of time? I was left alone for a while, but it wasn't too long

You can tell that he/she really makes an effort to look professional Did you leave the office feeling satisfied with your visit? Completely! Was this provider willing to stay open late to accomodate your needs? Yes, they've stayed open late when I've needed help Would you refer this provider to a family or friend? Yes

Yes, I received sample toothbrushes and toothpaste Does this provider treat their staff well? Yes, they treat their staff with respect Does this provider welcome questions? Yes, they don't mind answering my questions Was this provider's office clean? Yes everything was neat and sterile

Not at all, they rushed me out the door Did this provider leave you unattended for an extended period of time? No, I was always attended to Did you notice a foul odor when you arrived at this provider's office? Not at all, their office smelled fantastic!
