AVOID THIS DOCTOR!!!! He is a scam and will not provide a tax id for insurance because he does not want to report his income.
Business response:
As I explained to this person,I am not a provider on any insurance company rolls and I do not provide my tax ID to insurance companies because I do not accept payment from them.(When I provided my tax ID in the past I found that insurers listed me as paid by them which falsely made it look to the tax people as if I had been paid twice in each case.) As an out of network provider I help fill out forms for the patient's payment . As almost always happens, the patient was reimbursed by her insurer.
