I tried making an appointment a while ago and sent in the new patient packet. I ended up making an appointment and having to cancel shortly after. Apparently in cases like that they throw out your new patient packet but when I called to reschedule my appointment nobody mentioned that would happen. They only told me I would hear back in September. I called back today for updates and that’s when they told me they threw out my new patient packet and I would have to fill out a new one.
