Did they follow up with you after your appointment? Yes. Was it easy to make an appointment with this provider? Yes Was this provider's office clean? Yes everything was neat and sterile Does this provider have a good professional reputation within your community? Definitely! Everyone knows they are the best around! Was this provider's staff friendly? Yes

No, they take every concern of mine seriously Were the staff members able to answer your questions? Yes, they were able to answer my questions Did this provider answer all of your questions? Yes, they were willing to ask for advice when necessary Did this provider have an entertaining selection of waiting room magazines? Yes Were the restrooms clean at this provider's office? Yes, I could tell they were cleaned regularly Was this provider's office clean? Yes everything was neat and sterile Did you spend a lot of time in the waiting room at this provider's office? The wait wasn't terrible, but it wasn't the best

No, I felt confused and uneasy when I left Was this provider's office easy to locate? Fairly Easy Are you going to visit this provider again? Not if my life depended on it
