Did this provider thoroughly explain the risks and benefits of your treatment? Yes, I felt informed when I left Was this provider late to your appointments? Not at all, I showed up early and they were ready to see me Did you feel safe in this provider's care? Absolutely! I knew I was being cared for by an expert! Were the waiting room chairs comfortable at this provider's office? They were average Does this provider always take that extra step to make you feel special? Yes, I always enjoy my appointments

Were the waiting room chairs comfortable at this provider's office? Not really Did this provider ever take personal phone calls while in the appointment room with you? Yes! He/she took a personal call right in front of me. I found it to be incredibly rude. Were the restrooms well-stocked at this provider's office? No, they needed to make more of an effort to stock them

Has this provider ever cancelled your appointment on you last minute? Never, they are very reliable. How would you compare this provider to others in his/her field that you have visited? Fairly Average Does this provider leave you on hold for a long time when you call? Never! I don't think I've ever been put on hold here for a long time Did the staff make you feel uncomfortable when you called with questions or concerns? Not at all! They were happy to address every concern I had
