Been off work for a month now because I can't get a doctor's note from them or receipt I was told it takes one or two days a few times and yes sir I was promised I would get it by the end of the day and still nothing

My appointment was weeks ago so it pretty hard to remember the details. I was expecting an email with directions as to what i should try for the pain but never got one. The doctor suggested something new but I have no idea what it is called
