Did this provider promise services he/she couldn't provide? No, they were able to provide everything they said they would Did you receive sample toothbrushes or toothpaste to take home? Many! I got to pick my favorites from a huge selection Does this provider always put the customer first? Yes, they always put customers first Were the staff members able to answer your questions? Yes, they were able to answer my questions Did they follow up with you after your appointment? Yes.

Yes Are you confident that this provider will continue working with you until a solution is reached? Yes, I know I can count on them to find a solution Did this provider answer all of your questions? Yes

Does this dentist use current and safe equipment? Yes; the equipment is very advanced! Did this provider have reasonable prices? Absolutely! Did this dentist pressure you to get any unnessary dental procedures? Not at all! They always have my best interest in mind

Does this provider maintain a professional appearance? Yes, he/she looked very professional Did a staff member acknowledge or greet you when you entered? Immediately! They gave me a friendly greeting as soon as I walked in Does this provider remember you by name? Every single time, even if it's been months since my last appointment!! Did this provider wear an obnoxious cologne? No, they didn't wear any cologne
