DO NOT COME HERE - if you are lucky enough to secure an appointment, you will receive a call 1-2 years later claiming you owe them some ridiculous amount of money for general services. They'll tell you that your insurance won't cover it, and that YOU need to call them to sort it out. They have now done this to me twice - I am being hassled for payment from Oct 2017.
