My husband was referred to Choice Health for in-home PT. He had hip replacement surgery complicated by the need for two revisions and another serious, life-threatening illness that cropped up during his third recovery. He was supposed to have two sessions of PT a week after rehab for the second revision and has ended up with a total of two sessions in almost three weeks and nothing scheduled for later this week. Unfortunately had to file an abuse claim with medicare medicaid . Filed a police report as well and I have video footage of the attack. Beware , once the abuse was seen. Nothing was done , if you have a video of abuse and nothing is done, that is a clear violation of medicare medicaid policy. Again: I have a video and nothing was done. Please protect your elders. If you would like copies of the video, email me. Im not hiding it , its public .

When my mother was discharged from the hospital, we were told that Choice Health at Home (formerly Alliant Home Health) would be taking over her wound care. Unfortunately, our experience with this agency was extremely disappointing. My mother was on a wound VAC and needed to be seen by a home health nurse at least twice a week. Don't use this company. I have been trying to get PT to come out and work with my mother for 3 months. They never show up. I have talked the company and Heather, the scheduler says things are taken care of and PT will be scheduled, it never happens. I had OT restarted for her, and nothing has happened. Alliant is NOT reliable. Just a lot of wasted time, and sone guy named Steve canceled my mother's pt, oit of no where. Don't use this company.

This has been one of the worst home health companies I have experienced. There appears to be a serious lack of accountability, and patient care does not seem to be the priority. Patients are accepted onto service, but visits are delayed or never happen. Nurses frequently do not show up as scheduled, leaving patients without the care they were promised.
