We called to make an appointment and was given one for 1 pm the upcoming Saturday at 1 pm. We were called by the office that a 930 spot had opened up— I tool it! Arrived at 9:20 and unfortunately we DID NOT fill out the form that WAS sent- our fault.. The staff Monique was very helpful in accommodating us . The insurance was verified We filled the forms , but within 10 minutes we were seen!!!
