Did this provider answer all of your questions? Yes Did this provider ever postpone your appointment? No, they never postpone my appointment

No, my appointment started at the scheduled time Were the waiting room chairs comfortable at this provider's office? Yes Did this provider thoroughly explain the risks and benefits of your treatment? Yes, they made sure I had all the information I needed to make a decision Did this provider promise services he/she couldn't provide? No, they were able to provide everything they said they would

Was it clear that this provider takes pride in the appearance of his/her office? Definitely! It was one of the best looking offices I've ever seen! Were the staff members able to answer your questions? Yes! They were the most knowledgeable staff I've ever seen! Did this provider use excessive foul language? Not at all! They are always respectful and courteous! Does this provider treat their staff well? Very! I've never seen an employer treat their staff better

Was this provider late to your appointments? Not at all, I showed up early and they were ready to see me Does this provider promptly return your phone calls? Yes Does this provider maintain a professional appearance? Absolutely! You can tell that he/she really makes an effort to look professional Does this provider remember you by name? Every single time, even if it's been months since my last appointment!!
