We love Beth! Sending the 'personal history' by mail could have save us time, instead of taken 15 minutes to complete it at arrival time. Unfortunately, coming before our 1:00 PM appointment wasn't an option for us.
I had no problems with my dental appt and have already made my 6 month visit.
