Re: Temple office. They started our engagement with the mailing of a packet they ask you to fill out before your appointment. Stapled to the top of that packet is a piece of paper with patient name, date of birth, and appointment time and date. Shouldn't be problematic unless your mailing is addressed to another person. I received a mailing for someone that was not me and daresay that if I opened the envelope that was first mailed to me, it would have been a HIPAA violation.
