Reviewed my notes from last visit and again there were flat out lies about what services were offered to me and my behavior and demeanor during the visit towards staff. This is nothing new since the VA pretty much falsifies notes about almost every visit with them. On top of that, the VA again missed my cancer diagnosis for the second time in two years.
