I went to the U of L Travel Clinic in June of 2015. Many months later, I received a bill. It took me a while to figure out that this must have been related to the Travel Clinic visit since there was no mention of this detail on their bill. The bill provided no phone number and no link so I could pay it online, so I had to waste a stamp to mail my check to P.O. Box 740041. According to my bank, my check was cashed on 10/22/2015.
