Unfortunately I went to a facility owned by them. A staff member escorted me and another client thru a doorway prohibited to clients where a welcome mat was placed in a position that created a hazard . Usually staff would remove it. This particular staff member entered first and failed to warn us or remove it , I ended up tripping over it an falling down a flight of stairs suffering injuries that I'm now stuck owing for medical treatment.
