Several issues with this facility. 1. Not attending to patients for over an hour in their own piss and feces for over an hour. No cover over a. 4 gang outlet 2. PHI information left in patient’s room - of all the pain the floor 3. Leaving the call bell outside of the patient’s reach causing the patient out of bed. 4.
