If you are a veteran with health care through the VA, do not go here. They will not contact the VA to get authorization as they are supposed to, and will try to stick you with the bill. If you contact the billing department, they send you back to the VA or the Clinic. If you try to get them to fix it, they will tell you "Well now it's too late.
