Touching Hearts at Home in the Capital District provided services to my 94-year-old mother who had just undergone hip surgery. First, Touching Hearts was only one of two agencies recommended by the hospital's discharge department. Within 24 hours of being notified one of their Managers came to the hospital (30 minutes away) and interviewed my mother to ascertain her needs upon discharge.
