My father was placed in respite care at Avir of Dallas on March 19th and 20th 2026 at this facility. He was returned to his house on Saturday morning the 21st soak and wet. He said he doesn’t recall being fed nor bathed. He has a g-tube so he could have been fed and didn’t know it. The facility lost his DNR paperwork along with his informational binder and the clothes that he wore to the facility.
