Edit: I was told to come up to 3001 Armand connector to fill out paperwork in person. I was not given a link or QR code to fill anything out digitally. No one is answering the phone today and it is within working hours Submitted paperwork for my child in April and never heard back from them. 3 months later, I'm hearing from our doctor they've been "trying to contact me" but I obviously hadn't received any calls.
