New patient, first office visit and Drs. Assistant couldn't get needle in my mom's vein; she yelled out profanity in frustration. Another assistant had to complete the task. Same day given copies of (another patient's) medical documents in error when making copy of mom's prescription listing. Assistant emailed the incorrect medical treatments the Dr. wanted us to review.
