As for dental care- excellent. As for your new document on financial responsibility of the patient, it strikes me as a bit of a non sequitur. First iit requires the patient to sign over dental insurance benefits to the practice. That is followed by a strong statement that the relationship is between the patient and the practice- and emphasizes that the insurance provider has no role in that basic relationship.
