I saw Dr. Weigert as my primary care provider while I was on short-term disability. During my first visit, she went through my disability paperwork with me, and I paid the $40 fee to have the necessary forms completed. I was told that this fee covered the paperwork needed for my disability, including my return-to-work form, which I felt was fair.
