Kathryn Donaldson called after receiving letter stating costs for proposed appointment would be likely $1,000.00. Asked for supervisor after being denied an opportunity to try out the testing first. Told no. Asked for a supervisor. I was given a LPN nurse who said she is like a manager after 27 years. Then told I would need to be transferred again to Jamie Farr. Left VM.
