Poor service. Went for rehabilitation after back surgery. Arrived in the evening over the weekend. I wanted something to eat and was the kitchen was closed and they have no food. Asked for some pain medicine and was told they could not access and that it may be Tuesday before I could get anything. Would not recommend this place for anything... if you just have to go make sure you have family with you at all times.

We picked this place because my mom had been hospitalized and needed in patient rehab. I would not send a loved one here unless rehab was needed for joint replacement or a fracture. They are great if you aren't medically sick. My mom passed because the medical staff was incompetent in my opinion. They failed miserably and the only reason 911 was called was at a family members instance. Even the ER doctor asked why they didn't send her to the hospital 12 yours ago. I selected Clearvista Lake Health Campus for my mother to come for rehab after experiencing a traumatic fall this year. From my first meeting with the staff members giving me a tour, and now moving my mother to the assisted living section, I have experienced nothing but professionalism beyond measure. You are greeted with smiles and friendly hello's from staff members as soon as you enter the facility. Not enough staff to take care of patents needs. My mother was not taken care of. Recrding keeping is not accurate. Some of the aides do not wear gloves when checking depends.
Excellent place to bring your family
Our mom was in rehab for her heart and skilled living after her release. I would recommend both! The nurses and therapists were professional, caring and listened. The food was creative, fresh and homemade. Ice cream 24/7! From the administration to the residents...all sweet and caring.

I have a very close relative at Forest Glen. Each time I have visited her, the facility is in great shape. It is always clean and the staff is extremely helpful. The staff treats everyone with respect. I have been to several of the brunches that I believe are held once a month. The food is very good and the service is exemplary.
