Melissa Johnson is a nurse navigator of the highest caliber. She helped me with my husband, who was suffering from dementia, and took him to physician appointments, advocated for him, provided detailed background to the doctors and detailed notes back to me. It was a lifesaver. She and her team of nurses managed medications for my husband, and provided tools to help make my life easier. I am a past client of this provider My Mother and I contacted Senior Care Management Solutions to engage some basic home care for Mom based on recommendations from her senior living community. While battling end stage COPD and cancer, initially Mom needed help with bathing and meal prep, but soon assistance was needed with all aspects of daily living. I was very pleased with the services and the person provided from this company. They were a huge help with my husband after his accident. Janeth was a wonderful home health aide and I was very sad to say good-bye to her and I hope to have her in the future or someone just like her.

We found the perfect care management solution for my Dad when we turned to SCMS. SCMS worked with me long distance to provide a beautiful CNA named ROSE who really got my Dad and his out-of-town family. Thanks to Melissa effectively managing my Mom's care, I have been able to go back to being her "daughter" for the last three years.
