I signed a form online fore a FREE checkup and signed the SDF form as I seen nothing invasive or COSTLY in the form then I see a bill for $1200 to my dental insurance for SDF treatments on 6 of my child’s teeth. I was not made aware there would be a cost as it was advertised as FREE from the school and the company.
