Dr Petty has decided that the laws governing PHI don't apply to him, so when you ask for your test results, you won't get them until Dr. Petty decides to grant you access to them. You will still pay for them, they are still your PHI, but Dr. Petty determines when you get to know the results, and you'll plan your life based on his schedule. Source: I went to this clinic 3 times to obtain my results and was outright refused, contrary to the literal laws governing PHI. Well done!
