Her name has changed to Samantha Horatshki, avoid at all costs. She will cancel appointments last minute, leave you without needed medications, allow staff to act inappropriate, will lie about phone calls, msgs, etc. In my opinion she should probably have her license revoked.
What a joke! She is not very bright! I met with her on 11/26/24 to establish a new patient at OSF. I have had problems with this Office before. And when I met with Ms. Hollister I told her not to start and or create problems. The Staff at her Office and all white ladies who are racist, unprofessional, rude and down right nasty!
