This office also operates under Mira Health. Extremely disappointed. I paid $400 for a yearly membership that was supposed to provide after-hours access to the doctor and the doctor’s direct contact information, neither of which I ever received or used. When I enrolled, Kimberly specifically told me I could request a refund within 30 days, which I did well within that period. Horrible
Dr. Siudmak is a wonderful doctor, but the contradictory and unlawful policy has me forced to restart care. Patients please be advised you are not required to agree with conflicting policies that fail to align with law. I have every intent to pursue legal action for their policy putting my health care at risk. Go to a doctor's office that focus on healthcare and not greed.
