She obviously doesn't notate a majority of what is discussed or answered. I've had to reanswer everything as if it was my first time talking to her. Had to remind her on two separate occasions about a rx she was supposed to send out & after 2nd session she didn't notate or send another rx she said she would. I have to fire her and seek someone that actually does their job. Frustrating to put it nicely. Seek elsewhere to save yourself issues.
