(1) It is very hard to find a parking space around this building. (2) You need to sign a form which states the difference between the physical exam visit and regular visit for medical issues. It says that, if you have any medical issues during your physical exam visit, you may get charged for both physical exam visit and regular visit. I NEVER see such a form at other doctor's office. My first interaction with this group is bad. Went ahead of appointment and got a fumbled claim from receptionist. Looks like when doctors don't want to see more patients they claim that they tried to call but couldn't leave a message to reschedule ... how lame is this old excuse in this day in age?!
